AN EASY PROCESS

ADMISSION GUIDELINES

MEDICAL                                              ​

A thorough history and physical examination will be completed upon arrival by our medical staff.  If you have had a physical examination within the last 30 days, please provide a copy for our admissions counselors.​

PHYSICAL                                      ​

An individual will be expected to actively participate in all drug and alcohol addiction treatment program activities and should be able to function without major limitations.

SPIRITUAL

We believe that recovery involves spiritual healing and ask that all individuals seeking help at Project Surrender are open to exploring this aspect of the program.

PRIOR TREATMENT

Knowledge about and availability of prior treatment episodes, including records and information, will be beneficial to staff reaching out prior to treating each individual.  Appropriate Release of Information (ROI) forms will be provided.

PRE-ADMISSION

Please take moment to fill out our free Pre-Admit Intake Assessment form. This information will help us provide you with the best, individualized treatment program tailored to your specific needs.

EXCLUSIONARY CRITERIA

  • Physical limitations which limit the individual's ability to participate fully in our program.

  • Severe psychiatric disorders in need of acute hospitalization.

  • Severe sexual disorders.

  • Chronic aggressive behavior incompatible with a group environment.

  • Other severe disorders in need of different treatment modality to be determined by Admissions Counselors
